You can create a venue by going to foursquare.com, signing into your account, and then searching for the venue you want to create in the search box at the top middle of the page.  You will be redirected to the Explore page where a list of possible venues will appear to the left of the map. If your venue does not already exist on Foursquare, you can create a new venue visiting https://foursquare.com/add_venue

You can also create a place on your phone by tapping the "Check In" button in the Foursquare app, searching for the place (this step is important!), and then if you can't find it, scroll down to the bottom of the list and click "Add this place."
